You may qualify if you are:

• 18 Years or older
• Have an A1C between 7 and 10%
• History of Cardiovascular Problems (Stroke, Heart Attack, Peripheral Artery Disease, Stent Placement, etc)

Do you have diabetic foot ulcers?

Do you have an ulcer on the top or bottom of the foot that has been present for 4 to 52 weeks?

To qualify you must be:

• Diagnosed with Type 1 Diabetes for at least 1 year,
• Taking a stable dose of long-acting insulin (Lantus, Levemir or Detemir)

• Diagnosed with Type 2 Diabetes for at least 1 year,
• Taking only Metformin, or 2 or more oral antidiabetic medications
